IMPORTANT NOTICE:

3.  All computations are for illustration purposes only. These do not constitute an approval nor an offer
     by BDO.

4.  An Installment Availment Fee of P250 will be charged for every approved Balance Transfer application
     and will be billed and reflected in the Cardholder's Statement of Account.

5.  Balance Transfer applications are subject to the approval of BDO.  Once approved, the Terms and
     Conditions Governing the Issuance and Use of BDO Credit Cards, the Balance Transfer Terms and
     Conditions, and the prevailing fees and charges shall apply.
